The content of this report provides the basic information, data and outcomes that show the Head Start commitment of resources to children and families in Northwest Wisconsin.
If you are interested in enrolling your child in any of the programs offered, please complete a child application and return it to Family Forum, Inc., 1507 Tower Ave., Suite 312, Superior, WI 54880. After the application is received, someone from the agency will set up a time to meet with you to do income verification to determine whether or not your child is income eligible for the program. A families income needs to be verified or documented as having zero income on the application according to the Poverty Guidelines. There are many ways, besides meeting the poverty guidelines, to determine if a child is eligible for Head Start. This will be determined at the time of verification.

Significant dates that may apply to your child’s schedule. Please review and make note of relevant dates on your home calendar so that you know when your center may be closed, have a “No Visit Week”, or when school at your center starts and ends throughout the year. This manual contains agency policies that are in place to protect and ensure that your child is safe and secure when in our care, and when your family is receiving services from Family Forum, Inc. Head Start. Please use this manual as a reference to our rules and regulations to help you understand why we do what we do. Please refer questions first to your child’s teacher or home visitor, and if more information is necessary, then contact the center manager.
This document contains the addresses of all of our centers, along with the names of each center’s managers, their email addresses, and phone and fax numbers. Also included here is contact information for all administrative staff.
